Abstract

Background : The properst aging of advanced gastric cancer has crucial role in determining resect ability and oper ative method, to prevent unnecessary oper ation and to predict the prognosis and survival rate. Although marked impr ovements have been made in computedt omogr aphy (CT ) technology and endoscopic ultr asonogr aphy (EUS) in recent years, reassessment of the lapar oscopy for gastric cancer is required as a preoper ative st aging tool. Therefore, we determined the usefulness of lapar oscopy for st aging of advanced gastric cancer without dist ant metast asis in CT and EUS.
Methods : St aging lapar oscopy was performed in 48 patients with advanced gastric cancerst aged T3 or T4 by CT and EUS. Laparoscopy was carried out with the patient sunder local anesthesia, and included visual inspection of abdomen, and biopsies for suspicious metast atic lesions. Lapar oscopic result s were compared with the post operative pathologic findings .
Results : Lapar oscopy was performed successfully in 48 patients. Lapar oscopy disclosed unrecognized distant metast ases in 7 patients (14.6%) judged to be eligible for potentially cur ative resection by CT and EUS. Preoper ative lapar oscopy showed an accur acy of 74% for serosal infiltr ation.
Conclusion : Our results suggest that lapar oscopy is an effective means of evaluating resect ability of advanced gastric cancerst aged T3 or T4 and can provide valuable help in planning surgical approach.
